Who is eligible to receive the Vaccine? All Northern Humboldt Union High School Students ages 16 and older, their family members ages 16 and older and Northern Humboldt Union High School Staff and their family members ages 16 and older
 
Which Vaccine?  Pfizer
 
Where on the MHS Campus? In the Big Gym, with the entrance and parking in the Southeast Parking Lot
 
When?  From 1 pm to 4 pm with the last people being checked in at 3:30
 
What do I bring?  Anyone receiving the vaccine must fill out the forms attached to this page:
  • The Open Door Consent to treat; please note one side of the form is for the adult patient, and the reverse side is for the parent to sign to consent for treatment of minors.
  • The Patient Registration Form
  • The Covid Vaccine Consent Form.  This again needs parental signature and includes important medical questions.
  • The Pfizer vaccine information sheet. 
You may print out the forms from this page or pick them up at any of our school's offices.  Please complete the forms for each person receiving a vaccine and return them to your school site by Wednesday May 5 by 1:00 pm. You may also fill them out at the Big Gym on the day of the vaccine but that will prolong the time it will take to receive your shot.
 
Will my Parent/Guardian have to be with me to receive the shot?  No, as long as you have all the forms completed and signed they do not need to be with you to received the vaccine.
 
Will there be a cost to receive the Vaccine?  There is no cost, Open Door asks that if you have coverage to bring your insurance card but please note that a lack of coverage will NOT be a barrier to receiving the vaccine.
 
What is the time commitment?  There may be a line as people check in, once you receive the vaccine you will asked to wait in the bleachers for 15 plus minutes.   This is a good reminder that if your paperwork is already filled out and submitted the process will go much faster.
 
If I have already had one dose of Pfizer but I would like to get my second dose at MHS can I?  Yes, just be sure to bring your vaccination card from your first dose.
 
Will there be a follow up date to receive my second dose?  Yes, Friday May 28 at McKinleyville High School from 1 to 4 ppm
